GeneraI Information
Medications are to be brought to the school nurse by the student's parent in their origional container. All medications must be accompanied by a note from the parent. Medications that are controlled substances must also have a note from the physician.
The physician can use their own form and fax to Debbie Draper @ 838-3671 or print this school form.
Scheduled Medications
These are medications to be taken daily. I arrange a time with each student and their teacher if necessary to stop by for their medicine.
As Needed Medications
These medicines are taken only as needed in the student's judgement. They are supplied by the parent and kept in the Health Center. They will be kept throughout the school year. Any not picked up by the end of the year will be destroyed.
School Supply Medications
The school supplies acetaminophen, ibuprofen, Tums, and Benadryl. A parent or step-parent will be called each time the student requests these medications unless it is on the same day and at least 4 hours later. If you anticipate your child coming in for these on any particular day, feel free to call on that day to give permission.
Taking Medication Home
High School students may take their medication home only with written permission from their parent. The parent may also come to the school to pick up the medication themselves. |
